Please complete this online Claim Form as fully as possible. An incomplete Claim Form or a form with insufficient information may delay the processing of your claim.

Upon submitting this online Claim Form you will receive an emailed copy of the Claim Form that you have submitted and details of the documents that we require in order to process your claim. This email will be issued to the email address that is recorded in your online account, so if you have recently changed your email address please check that the address that we have recorded for you is up-to-date before submitting this form. Dive Master Insurance Claims Department will also receive a copy of your submitted Claim Form and will open a claim file and await hearing from you and receiving the requested claim documents. Important Notice: email all necessary documents, as treatment invoices, doctor or hospital reports as soon as possible, to avoid delays in processing your claim.
